In preparation for your first visit to our office, please bring with you the referral form from your doctor, your insurance card and a photo ID. A demographics form and a HIPAA (Health Insurance Portability and Accountability Act) form are located on our website. You can print and complete these forms before your visit.

Any questions about insurance can be directed to our staff at any one of our greater Washington, D.C. locations. A list of insurances (link to insurances) that we accept is available on our website. Some insurances will require an authorization or referral form on file.
